What semaglutide does
Semaglutide is a GLP-1 receptor agonist. GLP-1 (glucagon-like peptide-1) is a hormone your gut releases when you eat. It slows how fast your stomach empties, signals satiety to the brain, and supports insulin sensitivity. Semaglutide mimics that signal at a longer-lasting, weekly dose.
The clinical effect: you feel full sooner, you stay full longer, your appetite for high-calorie foods reduces, and over time your body weight comes down. Average weight reduction at maintenance dose over 12 months is 10 to 15 percent in clinical trials.
Semaglutide is not a quick fix. It requires monthly follow-up, titration over several months to reach maintenance dose, and ongoing treatment. Stopping the medication generally causes weight regain. We are upfront about this during the intake visit.

Every patient starts with a physician medical intake. Semaglutide is not dispensed without screening and never bypasses the safety criteria below.
Medical intake and contraindication screening
Personal and family history of medullary thyroid carcinoma, MEN2 syndrome, history of pancreatitis, severe GI disease, pregnancy or planned pregnancy, and other contraindications are absolute or relative bars to therapy. We screen carefully.
Suited for: Patients with BMI 27+ with weight-related comorbidities, or BMI 30+ without.
Titration schedule
Standard titration begins at 0.25 mg weekly for 4 weeks, then increases monthly to 0.5 mg, 1.0 mg, 1.7 mg, and (when needed) 2.4 mg. Slow titration minimizes nausea, the most common side effect.
Suited for: All starting patients. Some pause at lower maintenance doses when results are sufficient.
Monthly follow-up
In-clinic or telehealth check-in: weight, side effects, dose decision, and clinical status. Lab work as indicated. We do not "set and forget" GLP-1 patients.
Suited for: All active patients.
Discontinuation criteria
Mandatory discontinuation: severe persistent nausea, signs of pancreatitis (severe abdominal pain), gallbladder disease symptoms, suicidal ideation, or pregnancy. Elective discontinuation: patient choice with clear discussion of weight regain expectations.

What to expect
Intake visit
45 to 60 minutes. Medical history, contraindication screening, goals discussion, and consent. We do not prescribe at the first visit if anything in your history warrants further workup.
First injection
Walked through in clinic. We teach injection technique, rotation sites, storage, and side-effect management.
First 4 weeks
Starting dose 0.25 mg weekly. Nausea is the most common side effect; usually manageable with timing, hydration, and small meals.
Months 2 to 6
Monthly check-ins with dose titration. Most patients reach maintenance dose by month 5 or 6.
Long-term
Monthly to quarterly check-ins at maintenance. We discuss whether to stay on long-term, pause, or step down based on your goals and response.

A note on pricing
Compounded semaglutide pricing varies by dose and pharmacy. We provide a clear monthly cost estimate at intake. Insurance generally does not cover compounded GLP-1s; some patients use brand-name Wegovy or Ozempic through insurance with their primary care provider instead. Financing is available.
